Drivers License Reinstatement Assistance

 

Completing an evaluation and the recomended treatment is not enough when your drivers license has been revoked.  You will need to attend a Secretary of State administrative hearing with the proper documentation to qualify for reinstatement. We highly recommend meeting with an IOPC counselor who can guide you in the right direction to getting your driving privileges reinstated.

  • alcohol and drug evaluation uniform report
  • addendum for uniform report
  • alcohol and drug uniform report update (if original is more than 6 months old)
  • risk education certification of completion
  • treatment plan
  • discharge summary
  • treatment verification form
  • aftercare plan
  • continuing care plan
  • support system documentation

 

It's easy to see why many people are ill prepared to attend a hearing.  Most people will have their original evaluaiton, risk education certification of completion, and discharge summary at most. On top of that, most treatment providers and evaluators are not familiar enough with the entire process to effectively prepare a client for the hearing.  This leads to a cycle of frustration and some people will end up feeling hopeless.  That's where we come in.
